Accident: Defective mortar shells fell short during training exercise and exploded among forward positioned Marines. Some eyewitnesses noted that the defects could have been spotted if the ammunition had been more carefully inspected. Instead, the problem was not recognized until seconds after the shells had been fired; there wasn’t enough time to clear the forward positioned Marines.
